Use an 8-/12 x 11 sheet
(8-1/2" vertical) score marks: 1/2" , 4-1/4 & 7-1/2
(11" horizontal) score marks: 1-1/4", 4-1/2" & 7-3/4"
Cut to the score line on the 1-1/4" fold & fold down two triangles
